ID Set Up Requirements

Two forms of identification are required to open a PMB with Valley Postal. Both names on the identification documents need to match.

By law, any CMRA receiving mail on your behalf requires you to complete identity verification, which includes notarizing and submitting the official USPS Form 1583 which will be uploaded to the USPS Business Customer Gateway.
Notarization of the 1583 forums can be waived if signed in front of Valley Postal Staff.

Corporate ID - Must meet the following criteria*
- *Must be:
  - Clear Photograph of the bearer
  - Traceable & Recognized in the U.S
    Must be from a recognized corporation located and in good standing within the U.S.
  - Current & Valid:
    The ID cannot be expired. It must contain enough information to confirm your identity and be traceable back to you.

Home or Vehicle Insurance Policy
- Active- The insurance policy binder/ declaration page must be currently active and "in force" on the day submitted.
- Official Policy Declaration Page- Must show name and the insured address.
